Bladder cancer in Germany: trends in incidence, prevalence and mortality

How has the burden of bladder cancer (BC) in Germany changed over the last decade? The OncoCase team investigated this question using anonymised claims data from 19 German sickness funds with 6.3 million insured persons, provided by GWQ ServicePlus AG. The results were presented as a poster at ISPOR Europe 2025.

Patients with at least two outpatient or one inpatient BC diagnosis (ICD-10-GM C67) were identified. Incidence, point prevalence and mortality were assessed for 2012, 2018 and 2023, extrapolated (age- and gender-standardised) to the German population, and compared with nationwide registry data from the German Centre for Cancer Registry Data (ZfKD) at the Robert Koch Institute.

Key findings

  • Standardised cumulative incidence was 0.048% in 2012 and 0.040% in 2023.

  • Standardised point prevalence remained stable at around 0.24% (0.236% on 1 January 2013 vs. 0.235% on 1 January 2024).

  • The mean age of prevalent patients decreased from 66.7 to 65.6 years, and the proportion of female patients from 26.2% to 24.3%.

  • Mortality among prevalent BC patients, extrapolated to the German population, rose slightly from 5.64% in 2012 to 5.97% in 2023.
